What to Bring for Your Tax Appointment

The more complete your documents, the faster and more accurate your return — and the better we can identify every deduction you're entitled to. Print this checklist or bookmark it before your appointment.
Personal Information
- Social Security Numbers (SSN) or ITINs for you, your spouse, and all dependents
- Date of birth for you, your spouse, and all dependents
- Valid government-issued photo ID (driver's license or passport)
- Last year's tax return (federal and state) — new clients especially
- Bank account and routing numbers for direct deposit of your refund
Income Documents
- W-2 forms from all employers
- 1099-NEC or 1099-MISC for freelance, contract, or self-employment income
- 1099-INT for bank interest income
- 1099-DIV for dividend income
- 1099-B for stock or investment sales (brokerage statements)
- 1099-R for pension, IRA, or retirement distributions
- SSA-1099 for Social Security benefits
- K-1 forms from partnerships, S-corps, trusts, or estates
- Rental income records and related expenses
- Alimony received (divorce agreements dated before 2019)
Deductions & Credits
- 1098 Mortgage Interest Statement
- Property tax bills or statements
- Charitable donation receipts (cash and non-cash)
- Medical and dental expense receipts (if significant)
- Student loan interest statement (1098-E)
- Tuition and education expenses (1098-T)
- Childcare provider name, address, and EIN/SSN
- Educator expenses (teachers: up to $300 in unreimbursed classroom costs)
- Energy-efficient home improvement receipts
Self-Employed & Business Owners
- Profit & loss summary or business income/expense records
- Mileage log or total business miles driven
- Home office measurements (square footage of office and total home)
- Business asset purchases (equipment, vehicles, computers)
- Self-employed health insurance premiums paid
- SEP-IRA, Solo 401(k), or SIMPLE IRA contribution records
- Estimated tax payments made (federal and WV state amounts and dates)
Health Coverage
- Form 1095-A if you purchased coverage through the Marketplace
- Form 1095-B or 1095-C from your employer or insurer
- HSA contribution and distribution records (Form 5498-SA / 1099-SA)
Life Changes This Year
- Marriage or divorce — bring marriage certificate or divorce decree
- New baby or adoption — Social Security card for the child
- Bought or sold a home — closing disclosure (HUD-1 or CD)
- Started or closed a business
- Inherited money or property — estate documents
- Received a large gift or made a large gift to someone
Pro Tip from Danny
Don't stress if you're missing a document or two — we can work around it and amend later if needed. But the more you bring, the more we can find. Most clients who come prepared walk out with a larger refund than they expected.
